Carley Swanson, RN
Program Manager, Bridging Health and Home, Sanford Health

As program manager, Carley is responsible for the day-to-day operations of the Bridging Health and Home program. This includes facilitating communication between the community and Sanford, leading the Advisory councils, supervising staff and implementing and assessing strategies that align with the overall objectives of this pilot program. Carley earned a bachelor’s degree in Nuclear Medicine Technology followed by a research career developing innovative imaging technology for breast cancer. Through this work she sought a degree in nursing and has held positions in both large inpatient units and a Critical Access Hospital giving her a unique inter- and intraprofessional viewpoint.
 

Presenting at the Nexus Summit:
This interactive workshop explores the possibilities for interprofessional community building using lessons learned from the Bridging Health and Home program. This program intertwines a nurse-led interprofessional care delivery team, complemented by Faith Community Nursing, and evidenced-based self-management programs to improve the ability to age in place for rural older adults.
